Welcome to the 2017-18 Prairie Fire Season!

There are a number of forms that need to be filled out and returned to the athletic department before you can start practice or competition. Take some time now to learn more about forms, physicals, and insurance before you arrive on campus in the fall.

Physicals

You cannot participate in practice, competitions, and/or organized off-season training sessions without a yearly physical examination. Athletic physicals will be offered on campus the following days:

  • Football Athletes: Sunday, August 13, at 2:00 pm
  • Fall Sports Athletes: Friday, August 18, at 5:00 pm
  • All other Athletes: Wednesday, September 13, at 6:15 pm

The cost for the physical exam is $35 and will be charged to your account. All athletic physicals will take place in T. Fleming Fieldhouse and the basement of Memorial Gym. 

If you'd like to obtain a physical exam from your own physician prior to coming to campus, download the Athlete Physical form (PDF). All physical forms should be mailed to Scott Sunderland by August 1, 2017.

Insurance

In addition to an annual physical, all student-athletes must submit a new Parent/Guardian/Student Information form (PDF) annually, along with a copy of both sides to any applicable insurance cards. Please return your completed Parent/Guardian/Student Information form and insurance card copies to Scott Sunderland by August 1, 2017.

Knox College will provide a group accidental insurance policy for all Knox student-athletes. Learn more about Knox Athletic Insurance. It is important that you and your parent/guardian have a clear understanding of how personal insurance, student insurance, and the athletic insurance will cover an athletic claim. Questions about athletic insurance should be directed to Scott Sunderland or Beth Deuel, claims processor for First Agency, Inc. at 269-381-6630.

Sickle Cell Information

If this is your first time competing in athletics at Knox College, you must complete a Sickle Cell Trait Testing form (PDF) prior to participating in practice, competition, and/or strength and conditioning activities at Knox College. Please send your completed form to Scott Sunderland by August 1, 2017.

Minor Student-Athlete Consent for Medical Treatment

If you will not be 18 years of age a the start of the athletic and/or academic year, please fill out the Minor Student-Athlete Consent for Medical Treatment form (PDF). This document will be used to provide day-to-day care of the student-athlete by the medical staff in the Athletic Training Room at Knox College and in emergency situations by other licensed medical providers when the parent/guardian is unable to be contacted. Please return the completed document signed by all legal parents/guardians  to Scott Sunderland by August 1, 2017.
