Proof of immunity to certain vaccine-preventable diseases is required by state law for all new students enrolling at Knox.
To avoid the inconvenience of delaying registration for new students, visit your personalized student portal to complete your immunization form. Returning students do not have to resubmit their immunization records.
If you have difficulty completing the immunization form or locating your physician or your immunization records, you may find it helpful to contact your high school. Many high school immunization records satisfy the Illinois Public Law requirements. Should we require additional verification, we will contact you.
Please note the following immunization requirements:
- Two (2) vaccines against Measles, the first on or after your first birthday. The second vaccine should be received no less than one month later.
- Vaccinations against Mumps and Rubella on or after your first birthday.
- Protected against Tetanus/Diphtheria. The last dose of Tetanus/Diphtheria vaccine must have been received within the last 10 years. International students must show evidence of three (3) Tetanus and Diphtheria vaccines, one of which must be in the last 10 years.
If you need to complete the required vaccinations once on campus, please contact the Knox County Health Department. If you have any other questions regarding your immunization requirements, please call the Office of Student Development at 309-341-7222.
