Do you have your forms ready?

New and returning students must have emergency medical information and immunization records on file in order to register for classes or participate in sports. Both of these forms can be found on your personalized student portal.

If you plan to participate in varsity athletics at Knox, we will need a few additional forms. Download athlete physical, sickle cell trait testing, and insurance information forms and learn more about athletic fees and physical exams that will be available on campus this fall.
