Immunization Requirements

Immunization Requirements

Proof of immunity
to certain vaccine-preventable diseases is required by state law for all new students enrolling at Knox.

To avoid the inconvenience of delaying fall registration, we must receive your immunization form (PDF) by August 6, 2012.

State law prohibits any student who fails to supply satisfactory proof of immunity from being enrolled at Knox. In this event, you will be required to report to the Office of Student Development during registration to receive instructions on how to comply with this state law.

If you have difficulty completing the immunization form or locating your physician or your immunization records, you may find it helpful to contact your high school. Many high school immunization records satisfy the Illinois Public Law requirements. You may have your high school forward your records to us. Should we require additional verification, we will contact you.

Please note the following immunization requirements:

If you need to complete the required vaccinations once on campus, please contact the Knox County Health Department.  If you have any other questions regarding your immunization requirements, please contact Melanie Lowe, health services coordinator, at 309-341-7863.
