Health Finest Network (HFN) Cottage Group
List of all providers who offer a discount for utilization of medical services. In network providers are paid at 90% (after a $750 deductible). Out of network providers are paid at 70% (after a $1,500 deductible). In the client code box type KNOX. HFN has provided us with a tool to compare and research hospitals, physicians, home health care, and nursing homes. You may access this information by clicking on "Tools and Resources" (located on right side of screen, under "Quick Links"). Scroll down and enter login: KnoxCollege (case sensitive) and password: client4306 (case sensitive). Click on "Accept."

OSF Direct Access-OSF Group
List of providers who offer a discount for utilization of services with the OSF Group, which includes Prompt Care and the former Galesburg Clinic. In network providers are paid at 90% (after a $750 deductible). Out of network providers are paid at 70% (after a $1,500 deductible). 

PHCS Healthy Directions (MultiPlan)
List of providers located out of the OSF area who offer a discount for utilization of medical services.  In network providers are paid at 90% (after a $750 deductible). Out of network providers are paid at 70% (after a $1,500 deductible). The PHCS Healthy Directions logo is located on the back of your medical card.
